What is on your record

Your care, on one file

Your medical record holds your admission and discharge details, the clinical notes and diagnosis recorded by your care team, results from diagnostics and imaging, and a full history of every status update during your stay.

It also carries your itemised bill, so you can see every charge and what has been paid at any time through the patient portal.

Requesting records

How to request a copy

Have the Patient ID

You will need the Patient ID (e.g. HOS-2026-847291). It is on the admission slip or any message from the hospital.

Confirm who is asking

Records are released to the patient or a family member the patient has authorised. Bring photo ID to the records office.

Collect or receive securely

Discharge summaries are usually ready within a few days. Full records are prepared and released securely by the records office.
